End-of-life care is a deeply personal form of support that focuses on comfort, dignity, and peace during a person’s final stage of life. At HealthSense, we understand that to provide such compassionate and comprehensive care, we must spend meaningful time with each client and their loved ones—listening to their wishes, understanding their values, and learning who they rely on for support. Our process begins with a thoughtful, in-depth assessment that considers a client’s:
Physical Comfort and Care Needs | ensuring pain symptoms are well managed
Cognitive Changes | understanding how memory, clarity and decision-making maybe affected
Emotional and Spiritual Support | respecting beliefs, traditions, and sources of comfort
Medications | simplifying and adjusting treatments to prioritize comfort and reduce strain
Daily Routines | providing help with eating, bathing, grooming, and other personal needs
Special considerations | such as nutrition, skin care, or preventing discomfort from changes in mobility
Care environment | making the home or care setting safe, soothing, and peaceful
Support network | bringing together family, friends, and care providers to create a strong circle of care
With every step, our goal is to ease the burden on families, ensure comfort for the client, and create space for meaningful moments together.
We’ll design a care plan that brings comfort, dignity, and peace, while supporting your loved one and family every step of the way.
