Advanced Directive consulting is a deeply personal service focused on helping clients explore, document, and communicate their wishes for future medical care. At HealthSense, we provide expert, compassionate guidance through these important conversations, ensuring your values are clearly articulated and legally documented before a crisis occurs. Our role is to provide the education, facilitation, and support needed to give you and your family peace of mind.

Our process begins with a comprehensive Values & Goals Discovery session, which covers a client’s:

  • In-depth discussion of personal values, spiritual or ethical beliefs, and life goals as they relate to health care decisions.

  • Clear education on various life-sustaining treatments and their implications (e.g., CPR, intubation, feeding tubes).

  • Guidance on selecting the most appropriate Health Care Agent (Proxy) who can be trusted to honor your wishes.

  • Review of family dynamics to anticipate challenges and foster open, constructive communication about your decisions.

  • Exploration of specific wishes for various medical scenarios, such as terminal illness, permanent unconsciousness, or advanced dementia.

  • Assessment of any existing directives to ensure they are up-to-date and accurately reflect your current preferences.

Following our discovery session, we guide you in creating a comprehensive Advance Directive Portfolio. This is not just about filling out forms; it's about creating a clear and legally sound representation of your voice. This portfolio may include:

  • Completion of a Health Care Proxy to legally appoint your chosen agent to make medical decisions on your behalf.

  • Creation of a detailed Living Will that provides specific instructions and preferences for medical treatments you would or would not want.

  • Facilitation of crucial conversations with your physician to complete MOLST (Medical Orders for Life-Sustaining Treatment) and Non-Hospital DNR (Do Not Resuscitate) forms where appropriate.

  • A clear plan for communicating your wishes and distributing your directives to your agent, alternate agents, family members, and medical providers.

  • Recommendations for the proper storage and accessibility of your documents.

We also provide assistance in implementing and reviewing your directives, which may include:

  • Facilitating family meetings to explain your directives, answer questions, and ensure everyone understands your wishes and the role of your Health Care Agent.

  • Acting as a liaison between you, your family, your attorney, and your medical team to ensure all parties are aligned.

  • Assisting with the official distribution of your completed documents to physicians, hospital systems, and key family members.

  • Providing scheduled periodic reviews (e.g., annually or after a major life event) to update your directives as your health or wishes change.

  • Advocating to ensure your documents are located, understood, and honored in a healthcare setting during a medical crisis.
